When Should You Consult with a Doctor?
Many women are hesitant to speak about miscarriages, but they are devastatingly common. According to the American Pregnancy Association, "10-25% of all clinically recognized pregnancies will end in miscarriage."
A miscarriage is any pregnancy that involuntarily ends on its own during the first 20 weeks. When a woman suffers two or more involuntary first or second trimester losses, it is called a recurrent miscarriage. According to the American College of Obstetricians and Gynecologists (ACOG), 1% of women will suffer recurrent pregnancy losses in their lifetime.
The ACOG recommends scheduling an evaluation after three recurrent miscarriages to determine the underlying cause.
What Causes Recurrent Miscarriages?
Chromosome Abnormalities
The most common cause of miscarriages is abnormal chromosomal activity during fertilization. This can happen randomly, although there is an increased risk that comes with age.Physiological Conditions
Certain physiological conditions can lead to recurrent pregnancy loss. For example, a septate uterus divided in two by a wall of tissue can create an inhospitable environment for the embryo. Another condition known as Asherman Syndrome can lead to scarring in the uterus and rejection of the embryo before a woman even realizes she is pregnant. Other physical conditions including fibroids and polyps have also been linked to recurrent miscarriages. These abnormalities can limit blood supply to the pregnancy or cause dangerous inflammation.Preexisting Medical Conditions

What to Expect as a Patient at Our Clinic
Testing
If you have suffered three or more miscarriages, it is recommended that you visit a doctor. Our Omaha, NE, team can conduct testing to determine the cause of your recurrent pregnancy loss or provide treatment that helps you increase your chances of pregnancy. Our doctors can administer a medical evaluation, review your family history and genetic history, perform a pelvic exam, or order genetic testing to determine whether a specific condition is causing your recurrent miscarriages. Karyotypes may be performed for both the mother and father to determine whether genetic abnormalities are causing miscarriage.
Imaging
Our evaluation may also include an ultrasound or hysteroscopy, which helps our doctors evaluate the health of your uterus and uterine cavity. In most cases, we will begin with an ultrasound to evaluate the shape of the uterus and detect abnormalities such as fibroids. We may use hysteroscopy to view the inside of the uterus (the uterine cavity) and identify polyps, fibroids, uterine septums, or other abnormalities that could be causing recurrent issues. During a hysteroscopy, we can also perform minor surgical treatment to remove these abnormalities.
Treatment
If a specific cause is found, we can provide treatment to increase your chances of a successful pregnancy. However, it is important to note that many women show no cause for recurrent pregnancy loss. Depending on your diagnosis, our doctors can help design a treatment customized to your condition and goals. If chromosomal errors are to blame for pregnancy loss, we may recommend in vitro fertilization (IVF) with genetic testing. If physical obstructions such as a septate uterus or endometriosis are diagnosed, we may be able to perform corrective surgery to help you become pregnant.
Awaiting Results
It is important to remain patient and optimistic throughout the process, as recurrent miscarriages do not mean that you can never become pregnant. According to a study released by the University of California Los Angeles, the chance of a successful pregnancy even after pregnancy loss is approximately 77% for women who did not have any abnormalities during testing. If we are unable to achieve pregnancy through IVF or other ART procedures, we can explore additional options including egg donation and surrogacy.
